Each significant, hard-to-reverse decision is recorded as a short ADR: NNNN-short-title.md, numbered sequentially, never renumbered. A superseded ADR is kept and marked Superseded by NNNN.

Sections: Status (Proposed · Accepted · Superseded), Context, Decision, Consequences, Alternatives considered.
